Modern Face Deep Neck
Complete neck treatment with total submandibular gland preservation. Maximum safety without compromising results.
Non-Negotiable Principle: The submandibular gland is NEVER removed in Modern Face. Safety over aesthetic millimeters.
The neck is often neglected in facial surgeries, but it's impossible to have a harmonious result without treating it adequately. Modern Face Deep Neck addresses the neck comprehensively — platysmal bands, submental fat, hypertrophied digastric — but always within well-defined safety limits.
The fundamental difference in our approach: we never remove the submandibular gland. While some schools advocate removal to "clean" the mandible, we consider the risk unacceptable. The proximity to the marginal mandibular nerve, facial artery and vein, and hypoglossal nerve makes this maneuver unnecessarily dangerous.
The Four Pillars of Deep Neck
Non-Negotiable Principle
Total Gland Preservation
NEVER remove the submandibular gland. The risk of injury to vital structures does not justify the marginal aesthetic benefit.
Preservation Technique
Controlled Digastric Shaving
Reduction of hypertrophied anterior digastric belly by shaving, not complete excision. Maintains function, reduces volume.
Structural Definition
Layered Platysma Plication
Platysma plication with layered technique. Starts at the mentum, descends to the angle, returns. Creates cervical definition.
Durable Support
Lateral Crevassi
Platysma plication to the SCM fascia. Lateral support that prevents recurrence and maintains the cervical angle.
Indications
Modern Face Deep Neck is indicated for patients presenting:
Heavy neck with submental fat excess
Platysmal bands visible at rest or dynamic
Hypertrophied digastric (belly like 'a finger')
Loss of cervicomental angle
Moderate to severe cervical skin ptosis
Secondary cases with unsatisfactory prior results
When NOT to do isolated Deep Neck
If the midface is also compromised (malar ptosis, deep nasolabial fold), isolated Deep Neck will create disharmony. In these cases, combination with Endomidface is essential for a harmonious result.

What can be achieved
"Removing the submandibular gland to gain millimeters of definition is trading safety for technical vanity. Modern Face doesn't accept this trade-off."
