Main Indications

Midface Laxity

Cheek ptosis, loss of malar projection, early nasolabial fold accentuation.

Lower Eyelid Changes

Vertically long lower eyelid, negative vector, mild to moderate scleral show.

Patients with fixed lateral SMAS fibrosis. Superior access bypasses the risk zone.

Absolute Contraindications

  • Active smoking without prior pause (minimum 30 days)
  • Severe skin excess in face and neck
  • Jowls as main isolated complaint
  • Total baldness (for associated frontoplasty)
  • Essential blepharospasm
  • Patients over 70 with severe skin redundancy

What Endomidface Does NOT Correct

  • Marked jowls — mandibular line laxity is not corrected
  • Significant cervical excess — flaccid neck requires associated Deep Neck
  • Severe mandibular ptosis — may require pre-auricular incision
